Physical Therapist
We are looking for a full-time physical therapist to work in our Rehab department. The staff Physical Therapist is responsible for the initial evaluation and establishment of the treatment plan and goals in coordination with the referring physician of patients of all ages served: neonates, pediatrics, young adults, adults, and geriatrics.
The Physical Therapist performs physical therapy treatment, administers physical therapy modalities and therapeutic exercise, and provides patients and/or their families with education. Treatment includes our inpatient, outpatient, and swing-bed patients.
The Physical Therapist supervises and delegates tasks to the Physical Therapist's Assistant. The Physical Therapist will participate in team conferences/care planning. The Physical Therapist maintains effective communication with referring physicians. The Physical Therapist may serve as a clinical
instructor of Physical Therapist/Physical Therapist Assistant students.
QUALIFICATIONS
A. Education: Physical Therapist must be a graduate of an AMA-APTA-approved school of physical
therapy with a Bachelor’s, Master’s, or Doctorate of Physical Therapy or equivalent in Physical
Therapy.
B. Licensure: Must hold an active license for the state of Illinois or a permit from the licensing board
awaiting licensure.
