Occupational Therapist
JOB SUMMARY
The staff Occupational Therapist is responsible for the initial evaluation and establishment of the
treatment plan and goals in coordination with the referring physician of patients of all ages served:
neonates, pediatrics, young adults, adults, and geriatrics.
The Occupational Therapist performs occupational therapy treatment; and administers occupational
therapy modalities, therapeutic exercise, therapeutic activities, and ADL/IADL training; and provides
patients and/or their family's education.
The Occupational Therapist supervises and delegates tasks to the Occupational Therapy
Assistant. The Occupational Therapist will participate in team conferences/care planning. The
Occupational Therapist maintains effective communication with referring physicians. The Occupational
Therapist may serve as a clinical instructor of Occupational Therapist/Occupational Therapy Assistant
students.

ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Interpret physician referral for occupational therapy appropriateness.
2. Performs thorough initial evaluation including medical history, pre-existing functional status,
current baseline data regarding diagnosis.
3. Establishes specific treatment plan with stated frequency and duration and goals/desirable
functional outcome.
4. Reassesses patient and makes the necessary changes in treatment/goals for maximal
improvement.
5. Establishes and maintains effective communication (written/verbal) with the referring physician.
6. Appropriately delegates patient care to the Occupational Therapy Assistant.
7. Provides pertinent information regarding patient status and treatment to the Occupational
Therapy Assistant and reviews patient status with the Occupational Therapy Assistant on a regular
basis.
8. Provides adequate supervision to the Occupational Therapy Assistant assigned to the patient.
9. Determines when patient has reached maximum benefit from occupational therapy and
communicates this with the patient and referring physician.
10. Seeks additional information and/or clarification from the referring physician as necessary.
11. Explains treatment plan to patient/significant other before initiating treatment and answers
questions within own knowledge base or refers back to the physician.
12. Adapts treatment approach/plan appropriate to patient tolerance/progress.
13. Utilizes equipment/modalities according to established procedures.
14. Applies all phases of therapeutic exercise commensurate with education and training.
15. Maintains current knowledge of indications/contraindications and precautions of modality usage
and the population served.
16. Instructs, motivates, and assists patients to learn and improve functional activities, transfers,
ambulation, and ADLs.
17. Fits patients for, adjusts, and trains patients in use and care of orthopedic braces, prosthesis,
and/or supportive devices such as reachers, grabbers, sock aids, etc.
18. Documents evaluation and progress note in a concise and timely manner with content and
frequency per policy and procedure and maintains current treatment flowsheet.
19. Applies knowledge of developmental sequence/aging process when evaluating and performing
treatment to the :
a. Neonate
b. Pediatric
c. Young adult/Adult
d. Geriatric client
20. Requests assistance from colleagues in situations that exceed ability and makes necessary
referrals if unable to perform necessary treatment required.
21. Provides written instructions to patient/significant other for home programs.
22. Charges patients accurately and in a timely manner for the treatment received.
23. Serves as a clinical instructor for the Occupational Therapist/Occupational Therapy Assistant
student(s) as assigned.
24. Assists in new employee orientation as assigned.
25. Assists in clerical duties as assigned.
26. Assists in maintaining the department clean and orderly.

QUALIFICATIONS
A. Education: Occupational Therapist must be a graduate of an AMA-AOTA-approved school of
occupational therapy with a Master’s or Doctorate in Occupational Therapy or equivalent in
Occupational Therapy.
B. Licensure: Must hold an active license for the state of Illinois or a permit from the licensing board
awaiting licensure.
