Sports Physicals for junior high and high school athletes will be offered from 6:00 p.m. to 9:00 p.m. The $10 cost of the physical will be donated to each individual school represented. Pre-participation forms issued by IHSA will be available at the clinics. However, since this form serves as a consent, it must be completed and signed by a parent or guardian and presented at the time of the exam. Exams cannot be performed without the parent or guardian's signature. Please note that the exam is for SPORTS PHYSICAL ONLY and does not apply to school physicals. No appointment is needed.
